Almost everyone with scoliosis has been told to “strengthen your core”, and almost everyone has then gone and done crunches — which is close to the worst way to do it. A scoliotic spine is curved and rotated, and it’s held that way partly by trunk muscles that are working unevenly: overactive and shortened on one side, lengthened and under-used on the other, with the deep stabilisers often switched off altogether. Crunches train the big surface muscle at the front, add repeated flexion and compression to the spine, and do nothing about the asymmetry. What a curved spine actually needs from its core is control — the ability to resist being bent, rotated or collapsed further. Can scoliosis cause a weak core?
It can — though “weak” is only half the story. A lateral curve with rotation alters the leverage of every trunk muscle. On the concave side the obliques and back muscles are short and often gripping; on the convex side they’re lengthened and mechanically disadvantaged. Meanwhile the deep layer — transversus abdominis, the pelvic floor, the small multifidus muscles along the spine, and the diaphragm — tends to lose its timing, because the body has learned to brace with the big surface muscles instead. So the core isn’t so much weak as uneven and poorly coordinated. That’s the diagnosis your training should answer: restore deep-core timing first, then build strength on the side that’s lost it. It’s the same logic that underpins the wider set of exercises good for scoliosis.
Why crunches are the wrong tool
Three reasons, and each matters:
- Repeated flexion under load compresses the spine. The discs and the concave-side facet joints in a curve are already carrying more than their share. Hundreds of crunches add to that.
- They train the wrong muscle. Crunches target rectus abdominis, the “six-pack” muscle, which does little to stabilise a rotated spine. The stabilisers you need lie deeper and work best when the spine is still.
- They ignore the asymmetry. A symmetrical crunch lets the strong side dominate, so the imbalance that defines scoliosis gets reinforced rather than corrected.

How do I strengthen my core with scoliosis? The routine
Do these three to four times a week. Quality over quantity: a perfect 6 reps beats a sloppy 20.
- Breathing-led transversus activation — Lie on your back, knees bent. Inhale wide into the lower ribs, especially the concave side. On the exhale, feel the lower belly draw gently inward and the pelvic floor lift, without tucking the pelvis. 10 breaths. Feel: a light tension low in the abdomen, like a corset tightening a notch. Avoid: sucking in hard or holding the breath — this is subtle.
- Dead bug — Arms to the ceiling, knees over hips. Exhale and extend one arm and the opposite leg away, keeping the lower back quiet on the mat. 8-10 each side, 2 sets. Feel: the lower abdominals holding the pelvis still. Avoid: the ribs flaring; shorten the reach if the back lifts.
- Bird-dog — Hands under shoulders, knees under hips. Reach the opposite arm and leg long and hold the trunk level for 3-5 seconds. 8 each side. Feel: back and abdominals working together to resist twist. Avoid: the hip hitching up as the leg lifts.
- Side plank from the knees — Forearm down, knees bent, lift the hips into a straight line from knee to shoulder. Hold 15-30 seconds, 3 times with the convex side down, 2 times with the concave side down. Feel: the waist muscles of the underside lifting you. Avoid: the hips dropping back or the shoulder shrugging.
- Pallof press — Anchor a resistance band at chest height to one side. Stand or kneel side-on, hold the band at your chest with both hands, and press it straight out in front, resisting the pull to rotate. Hold 3 seconds, return. 10 reps each side, with 2 sets facing the direction that works your convex side harder. Feel: the obliques and deep core bracing against the twist. Avoid: leaning away from the anchor to cheat.
- Kneeling plank — On forearms and knees, body straight from knees to head, spine long. Hold 20-40 seconds, 3 times. Feel: even work through the whole trunk. Avoid: the hips sagging or the low back arching; drop the hold time before you let form go.
- Supine heel slides — On your back, knees bent. Exhale and slide one heel away along the floor until the leg is nearly straight, then draw it back, keeping the pelvis still. 8 each side. Feel: the lower abdominals resisting the pull of the leg. Avoid: the pelvis rocking side to side.
- Standing anti-rotation hold — Feet hip-width, band anchored to one side, held at the chest. Simply stand tall and resist the pull for 20-30 seconds, then switch. Feel: a quiet, whole-trunk brace. Avoid: clenching the jaw or holding the breath.
Progressing without losing control
Progress by making stillness harder, not by adding reps. Lengthen the holds, slow the tempo, move the band anchor further away, or lift a knee off the floor in the plank — one change at a time. If your spine starts to twist or sag, you’ve progressed too fast. The deep core is trained by resisting movement; the moment the spine moves to compensate, the exercise has stopped doing its job. When to stop: leg numbness, tingling or weakness, changes to bladder or bowel control, or pain that is clearly worsening rather than easing — pause and get assessed before continuing.
